Payment is due, in full, at time of service unless other arrangements have been made with this office.

Self-Pay:  Payment may be made with Cash, Check, Credit or Debit Card

Psychotherapy Sessions last approx. 50 Minutes

Self-Pay Session Fees:

$60.00 Individual Session

$80.00 Couples / Family Session

Insurance reimbursement rates based on individual contract

Individualized Coping Skills Training Fee:  $60.00 per Session

On-site Trainings Fee based on Contract

Court Testimony and / or Deposition: $200.00 per hour

Case Summary Letter:  $40.00

     I am contracted to provide out-patient mental health and employee assistance services as an in-network provider on the following insurance and managed care organization panels:

 Aetna - APS Healthcare - Blue Cross/Blue Shield - Cigna - Ceridian - ComPsych Corp. - Employee Services, Inc. - Health Partners of TN. - Horizon Behavioral Health - Magellan Behavioral Health - Managed Health Network - New Direction - Pacificare - Private Healthcare Systems - Tenn Care - United Behavioral Health - VMC - Value Options

(If your carrier is not listed, call the 1-800 number on your insurance card to verify in/out network coverage)

     ALL Employee Assistance Program (EAP) benefits must be pre-authorized through your workplace EAP Coordinator.

If you wish to use your insurance coverage for payment of session fees, please follow these instructions:
*
  • Call the number on your insurance card indicated for Behavioral / Mental Health Services and confirm with your provider that your member benefits cover outpatient mental health services.
*
  • Obtain from your insurance provider any pre-authorization that may be required for your insurance coverage.  Also, ask about your deductible and co-payment amounts.
*
  • After you have verified coverage and obtained authorization (if needed), therapist will file all necessary forms for payment reimbursement.
*
  • Clients will be responsible for all fees insurance benefit plan does not cover.
*
  • Payment for first session is required at time of service (unless other arrangements have been made) until insurance coverage is established.  Once coverage is established, any overage from session fee can be applied to subsequent co-pays as necessary.
